Abstract Click It or Ticket (CIOT) mobilizations are intense, short-duration, widely publicized enforcement efforts used to improve seat belt use. Annual national mobilizations have been conducted in May every year since 2003. National and State expenditures for publicizing the 2008 and 2009 CIOT mobilizations were at their lowest level since 2003 ($23.7 million in 2008 and $20.9 million in 2009) and the number of seat belt citations decreased each of the last three years. Telephone surveys administered before and after each CIOT campaign indicated increased awareness of police efforts to enforce the seat belt law and increased perceived risk of receiving a ticket if not wearing a seat belt, especially among males 18 to 34 years old. Statewide usage rates increased in 33 States (District of Columbia and Puerto Rico included) in 2008 and increased in 29 in 2009. The number of States increasing statewide belt use has decreased over time and the amount of annual increase was less than 1 percentage point for three of the last four years. It seems likely that more needs to be done to keep or extend gains made in seat belt awareness and belt use.
